Incident Report Process & Form

  1. Seek medical attention as needed, and keep all applicable information.
  2. Visit the College website and print the Incident Report form.
  3. Fill out College Incident Report (basic information of what happened, where, when, and how it happened).
  4. Send the completed Incident Report form to:

  5. An adjuster will then be assigned the case to investigate and determine if the College is/was responsible for the medical expenses incurred.
