Incident Report Process & Form
- Seek medical attention as needed, and keep all applicable information.
- Visit the College website and print the Incident Report form.
- Fill out College Incident Report (basic information of what happened, where, when, and how it happened).

- An adjuster will then be assigned the case to investigate and determine if the College is/was responsible for the medical expenses incurred.
